I've had a floor mat slide forward pushing on the accelerator before.

It aso helped trap a beer juice can behind the brake pedal.
Luckily I was sober smart enough to reach down and pull the floor mat back realizing something had obviously changed on the floor.

My current Toyota has a floor mat clip which disallows the mat sliding forward.

edit:
please substitute smart enough with mechanically inclined enough.

This happened locally, and was in the news for quite some time. Two things I remember from the coverage:

Turning OFF the engine would have required depressing the start button for three seconds. Unless someone told me that, I doubt I would remember such a thing, especially if I'm used to using an actual key to turn on/off the engine.

The driver was a California Highway Patrol officer for 20 years. If anyone might have had driver's training for emergency situations, I would hope that it would be a CHP officer.

No idea why he didn't shift into Neutral - any Lexus owners care to experiment and report back?

What I've learned from this - just give me a mechanical ignition and a stick shift, and I'll stay happy...
