I know who would be next in line if the president became incapacitated. What about the vice president's replacement?

The backup gets replaced with a new person. Not the next in line in the hierarchy.

If the latter was the case, then Pelosi would be W's VP if something happened to Darth Cheney

If president and vice president are both incapacitated control goes to the speaker of the house

This is purely for my knowledge.
I am not planning to do anything to Cheney. If anything it'd be his own undoing.
Back to the topic at hand, the VIP's replacement would be selected by the president or his party's majority?

Gerald Ford was the one selected to be the replacement for Nixon's original VP, Spiro T. Agnew. When Ford took over for Nixon they appointed Nelson Rockefeller to take Ford's place. So, it has already happened at least twice.

About Ford, from the Wiki:

He was the first person appointed to the vice-presidency under the terms of the 25th Amendment, and became President upon Richard Nixon's resignation on August 9, 1974. Ford was the fifth U.S. President never to have been elected to that position, and the only one never to have won any national election. He was also the longest-lived president in U.S. history, dying at the age of 93.

As billb noted, just look at the 25th amendment to the constitution. The relevant section for the VP:

Section 2. Whenever there is a vacancy in the office of the Vice President, the President shall nominate a Vice President who shall take office upon confirmation by a majority vote of both Houses of Congress.

As to the VIP, they're a dime a dozen. Why do we need replacements?
