If you are filling out a "Vendor Registration Form" for some work you were hired to do, what do you put for the name of your firm if you are just a guy who put a name to his freelance design business? I'm not incorporated or anything like that. Do you use d/b/a? Does it matter?

If it's for an expo registration, I usually make up a bogus company name. It helps keep track of the origin for the spam and junkmail that I get after each event. Appending "Designs" or "Design Studio" to your name would probably work nicely.

My invoices simply have my name on them. I'm not incorporated and consulting is a sideline for me. My understanding is that I don't need to register with the state for the kind of work that I do if I'm using my own name. (The tax situation is a PITA.)

DBAs are complicated. They usually need to be registered with the state and some states require publication of the registration in the local papers.
