anyone have a good tool to create web-based quizzes?

We're looking to create a 10-question quiz for our website, and would like to have results page that gives you a response based on how many points you scored. Something like 4 points for correct answer, 2 for an answer that's close, and 0 for a wrong answer,etc. Then your results page gives you a response based on how many points you scored:

0-2 points. You're not real smart

2-4 points. You're average

more than 4 points. You're a genius.

I've been looking at some free php scripts, and none of them seem to give me exactly what I need. Then I came across several commercial solutions, and they start at $500 or so. We're not going to spend that much for this quiz.

Ideally, the solution would be free or less than $50. I'm open to a php script solution, or even javascript. I might be open to a web service that offers a quiz service, but I would probably be less excited about it unless it was $5/month or something and could be embedded on our website (iframe??) so it's seamless to the end user.

any ideas?

It's actually a full blown survey program, but will do what you're looking for. I've got a couple of programs from this guy running on my web server, and they all perform flawlessly. He can even do the install, if you don't know how to do that sort of thing.
