Very sad and unfortunate. But it happens. Most probable cause was flammable anesthesia, oxygen, and a spark from equipment. Most people don't appreciate how dangerous gaseous anesthesia can be.

Under proposed tort reform she (well, her estate) would be able to sue the hospital to recover medical costs for those six days of medical expenses she incurred before she died. Which the estate would then repay to the hospital. No pain and suffering, etc. allowed. As a co-worker mentioned, she would probably be getting lots of free morphine so there wouldn't be any pain and suffering.
