I've been arguing with insurance, so has the doctor (him with multiple insurance companies) - can't get past it's the eyes. They hear eyes and say no coverage, maddening to say the least.

Is your doctor an ophthalmologist? If he/she isn't that might be the reason they will not cover it. I would suggest looking at your list of doctors, for the medical side, not vision, and see if there is an ophthalmologist listed that is in your network. I had a similar issue where I went to an optometrist for a medical issue with my eyes but it wasn't covered because he was only in their vision network and not their medical network, since he was only an optometrist.
