I've noticed my MacBook's adapter is starting turn sort white right where wire and the strain
relief meet at the magnetic end. I assume this is from excessive flexing. Has anyone tried
putting an additional strain relief over the little tiny short one that it comes with?

Some company came out with a simple cover to move the strain down the cable. I can not seem to find the link at the moment.

I don't think that they replace for what they consider "excessive" straining, they will replace them if they show signs of overheating and discoloration.

yes, the first generation had a thinner cable and was prone to these failures. AFAIK, Apple will replace these even out of warranty. Check with Apple anyway, what do you have to lose?

but wouldn't this item just move the problem about 1" down the line? there will be a new point of stress that will give up eventually
