You can apply online or you can mail your completed CMS 40B, Application for Enrollment in Medicare - Part B (Medical Insurance) to your local Social Security office. You’ll also need to send CMS L564 - Request for Employment Information, and a required proof of employment, Group Health Plan (GHP), or Large Group Health Plan (LGHP) coverage with your 40B form.
My online application also let me attach documentation of coverage by employer, but did not specify any official form. After submission, I received an email with a link to electronically sign the application, which I did, then I received a subsequent email stating You're done signing Medicare Part B Online Application
You were in a different situation, you already had Part A. When I was doing all of the research the SSA phone people were telling me that we could attach the forms to the online application. I later found out that only applied to people like you who were just applying for Part B and already had Part A. For us it was like a first time Medicare application and no way to attach anything online. I was also told that it would be processed at my local SSA office and I could FAX the forms there, but it turned out that online applications get processed in Richmond CA for my area and never go to my local office. I viewed getting the forms to the right person as the key to getting things processed quickly, and in the end we were able to talk with the person who the application was assigned to and email the forms.
If you didn't supply form L564 they might contact you, I think I remember seeing a list of other things they might accept if you can't get the form filled out, like paystubs or insurance cards.
