This PDF shows Monegan's travel request was signed 6/18, but it was a blank document and the date of "submission" (I guess that's what the 6/19 refers to) was the following day.

So hatchet falls on Palin's COS for signing a blank travel authorization. Apparently no one knew at that point that Monegan was going to DC to seek funding from an Alaskan Congressman. They used to call this "going outside the chain of command". He had no authority to negotiate funds for the state of Alaska.

Don't shoot the messanger, please. Just giving you the links and I will not discuss further.
