I don't think you always get the burning with acid reflux, I went to my doctor complaining of upper back and chest pains and one of the first things he asked me was if I'd been having a lot of heartburn lately, to which I replied no, he did diagnosis esophageal spasms caused by acid reflux and prescribed Prevacid which seems to be taking care of it.
