... the "f" bomb, referring to that he f'ed up ... they must not have realized that they were still on and live...
I'm guessing they get sort of fine for this?
That's the first time I can recall ever watching something live where this has happened... unfortunately I didn't have the DVR going, I was too shocked and it was like watching a slow-motion train wreck to see how long it'd take them to figure out they were still OTA...

People err.
There are plenty of example of that and the F-b on the Tube.
Given that 'fsck' is so prevalent in society vocabulary these days, I'd say it barely rates a mention.
My inner sexiest smiles when I hear a woman let it slip, on the bloopers or in actual life.

Back in the 70’s I had a show on our FM college radio station. My tag line was “WRUW ! Coming to you with less power than the average refrigerator light bulb !”
Thursday night, 11PM to 5 AM.
I did George Carlin’s “7 dirty words” every now and then.
The station manager yelled at me after she saw the log.

Wasn't it Charles Rocket who let loose an F-bomb on live TV years ago? Wasn't it a competing show of SNL called Fridays or something?

Yeah, I think it was Charles Rocket and yes, it was Fridays. That was the show in which I first remember ever seeing Kramer (Michael Richards).
And it was just a few years ago that Kristin Steward dropped it on SNL.

It was SNL, he was in a Who Shot JR Dallas skit, at the closing of the show with the cast on stage he said "I'd like to know who the fuck did it". I remember watching it live. It's also not clear that he got fired for it, there was a big shakeup and many cast members were let go.

The Saturday Night Live episode of February 21, 1981, hosted by Dallas star Charlene Tilton, featured a parody of the famed "Who Shot J.R.?" story arc from the then-popular nighttime soap. During the show a plot line had Rocket and Tilton flirting while other cast members expressed jealousy, leading to Rocket being shot in the chest by a sniper in the middle of a sketch. In the show's closing moments, as cast members gathered with the host to say good night, Tilton asked Rocket how he felt about being shot. In character, Rocket replied "Oh man, it's the first time I've ever been shot in my life. I'd like to know who the fuck did it."[8] He attempted to explain it away as an extemporaneous exclamation.
Due partially to the violation of broadcast standards, along with negative press regarding the new cast and declining ratings for both the series and the network in general, NBC replaced Doumanian with Dick Ebersol after one further episode. Ebersol, who placed the show on hiatus for a month to retool, dismissed Rocket, along with several of the writers and fellow cast members Gilbert Gottfried and Ann Risley, before the next episode. A writers' strike led to the suspension of the rest of the season, and when the show returned in October 1981, Joe Piscopo and Eddie Murphy were the only cast members who were held over from Doumanian's era.

Knowing a number of news anchors, I'm surprised it doesn't occur more often. Most could make a sailor blush. It was amazing how different they were from their on-air persona. We had a longtime anchor in Tampa who came across as straight laced as white bread, a favorite of the old-time conservatives. What none knew was that he was tattooed from the neck down, drank like a fish and couldn't string a sentence together unless f*ck was in it at least twice.

What happened in this case is that the weekend anchor -- who I don't remember seeing before as a weekend anchor, he looked like a very young kid, maybe in his mid-20's -- thought that he was off the air and was talking about how badly he'd screwed up giving the news. The background folks were chatting with him so it was obvious they were unaware too that they were still on air -- and some of what they were saying sounded very unprofessional though not profane. It went on for maybe 2-3 minutes and then faded out without explanation or anything. It was painful to watch.
