The image comes from a video Trump reposted of Trump supporters in pick-up trucks with pro Trump flags, etc. One of the trucks has the artwork screen printed on its tailgate.

I didn’t see any references to a ‘bullet hole’, and a higher res version of the image doesn’t seem to show one that I can see.
