I agree completely. Relying on a safety to be safe is sheer idiocy.
My KelTec P11 doesn't even have a safety. I rely on common sense and rigid compliance to safety rules.
Now, its entirely possible that the shotgun has worn components and a hard whack or dropping it set it off. But that's why you have the safety on and no round in the chamber until you are in place and ready to shoot. Even with the safety off, and defective components, it can't shoot without a round chambered.
