There's plenty of pre-made and custom made similar shelf options to be had. Price ranges vary as others have linked above.
I made another (semi-cheap) solution by carefully installing melamine clad cleats, above and below 3/4" thick shelves.
They (obviously) can't extend as fully as an articulated slide solution, but fairly cheap and easy was the rule of the day on that plebeian style.
