Depends on how careful you are, and your past track record.
In our home, we figure that phones get dropped more often, so they get AC+. Laptops usually don't, and so far we're in the black.

I have a 2017 MacBook Pro 15" that is suffering from a swelling battery. I've been waiting on an Apple Store to open up. They have a recall on some of the 2017's but when I put my serial number in the form, it says mine doesn't qualify. So we'll swe what they'll tell me to do about it when I do get to take it to them. I didn't buy the over $400 something AppleCare.
So... Maybe

Depends on which one. The new 16" has a number of new components, risk of some of them not lasting as long as one would like is high. I added AppleCare+ to my purchase.
The new 13" MBPR? Maybe not. The only real hardware changes are the updated CPU and the keyboard.

I would say without 2nd thought yes. Ssd goes, memory issue, gpu issue / all these things are soldered and non user serviceable. I think the extended warranty is a necessity on these newer systems - especially with the history of issues last 4 years.
